JOB SUMMARYUnder the direction of the Clinical Coordinator or Manager and the Pharmacy Director, optimizes patient medications therapy by providing comprehensive clinical pharmacy services. Works collaboratively with the health care team to accomplish goals.JOB REQUIREMENTSMinimum EducationB.S. Pharmacy or Pharm D, or Master of PharmacyContinuing education as required for licensure by TSBPMinimum Work ExperiencePrevious hospital experience with clinical responsibilitiesRequired Licenses/CertificationsActive registration as a licensed pharmacist in the state of TexasSterile Compounding CertificationACLS or PALSRegistration as a preceptor with TSBPRequired Skills, Knowledge, and AbilitiesAssess, document, and perform physician consultations, excluding total parenteral nutrition, but including all medication dosing consults.Perform home medication reconciliation.Provide education presentations to pharmacy staff and other staff upon request.Ability to effectively direct and/or supervise personnel as appropriate.Ability to handle confidential information.Ability to maintain professional appearance and decorum at all times.Ability to demonstrate excellent oral and written communication skills.Designated DriverNoOSHA Category1 - High RiskLocation: Hendrick Health Medication Management ClinicSchedule: Full Time, Day Shift, usually M-F, daysby Jobble
