Job Description:
Job Description Pride Health is seeking a Registered Nurse (Home
Health) to join our team in San Angelo, TX 76901. This is a 13-week
contract with the potential for extension. Both local and travel
candidates are encouraged to apply. Position Summary: The
Registered Nurse (RN) will provide skilled nursing care to patients
of all ages in their homes, coordinate care with the
interdisciplinary team, and take responsibility for the development
and evaluation of the patient’s Plan of Care. This position
requires strong clinical judgment, home health experience, and the
ability to work independently across various patient environments.
Job Responsibilities: · Assess home health and/or hospice patients
in both stable and emergent situations to identify physical,
psychosocial, and environmental needs · Document patient visits and
care accurately and in a timely manner using Homecare Homebase
(HCHB) · Provide patient and family education on care plans,
medications, diet, and home safety · Utilize infection control
measures and ensure OSHA compliance · Supervise Home Health Aides
and provide clinical direction as needed · Monitor pain, apply
appropriate management techniques, and educate patients/families ·
Make referrals to appropriate agencies and ensure third-party payer
compliance · Maintain effective communication with patients,
families, healthcare providers, and internal staff Required
Qualifications: · Graduate of an accredited Associate Degree in
Nursing or Nursing Diploma program (BSN preferred) · Current state
RN license (TX) · Current CPR certification · Minimum 2 years of
home health experience · Experience with Homecare Homebase and
OASIS documentation · Valid driver’s license, reliable
transportation, and proof of insurance Additional Information: ·
Location: San Angelo, TX 76901 · Schedule: Monday – Friday, Days
flexible hours based on caseload · Contract Length: 13 weeks ·
Travel Pay (Gross Weekly): $2,190 o Hourly W2 (Taxable): $23.60 o
Stipend (Non-Taxable): $31.15/hour (split: $19.25 housing $11.90
meals) · Local Pay Rate: $42 – $44.50/hour *Offered pay rate will
be based on education, experience, and healthcare credentials.
