Physical Therapist Location: Snyder, TX Agency: Cynet Health Pay: $2,526 per week Shift Information: Days - 5 days x 8 hours Contract Duration: 13 Weeks Start Date: 2/2/2026 About the Position Job Title: Physical Therapist Profession: Physical Therapist Specialty: Long Term Care Duration: 13 weeks Shift: Days Hours per Shift: 8 hours Experience: 2 years PT experience in LTC setting required License: Active TX or compact license required Certifications: None specified Must-Have: None specified Description: The physical therapist will accurately assess, differentially diagnose, and identify deficits in a patient’s ability to move and perform functional activities in their daily lives. They will develop an individualized treatment plan with patient-centered goals to reduce pain, restore function, and prevent disability. The therapist will work with the individual to prevent the loss of mobility. Essential functions include performing patient evaluations through the process of obtaining history, performing relevant systems review, and selecting and administering specific tests and measures to obtain data. They will establish an individualized plan of care and select treatment techniques that address the specific impairments identified in the evaluation and in accordance with physician orders. Ongoing reassessment of the patient and their response to treatment will be conducted. The therapist will initiate and manage the discharge plan beginning at the first visit and continuing throughout treatment. Supervision and delegation to ancillary staff and students will be consistent with state laws and departmental policy. Legible, accurate, and clear documentation will be completed within the established timeframe identified in departmental policy and procedure, in accordance with accreditation requirements and insurance regulations. Communication with the patient, family, caregivers, significant others, and members of the healthcare team will be essential to promote maximum benefit of care. The therapist will assess educational needs of the patient, family, caregiver, or significant other and provide education to meet those needs. Participation in professional growth and development through involvement in educational programs, including in-service meetings, workshops, and current literature reading, will be encouraged. The physical therapist will actively participate in quality improvement activities. An environment conducive to safety for patients, visitors, and staff will be provided. Compliance with appropriate and approved safety standards will be essential. The therapist will function within the scope of practice as defined by the state’s practice act.
