Job Description:
As part of our dedicated and compassionate team at Encompass
Health, the LPN-LVN II will play a pivotal role in delivering
exceptional care across our varied patient settings. Engaged in a
supportive and expanding network, this position provides an
opportunity to work both in facility-based settings and within
home-based care environments across 42 states. This role demands a
professional who is adept in nursing practices, patient
communication, and capable of functioning effectively in multiple
healthcare modalities, ensuring seamless service and continuity of
care. Provide direct patient care according to the established
patient plan of care Ensure the proper administration of
medications and monitoring of patient status Assist with the
implementation of physician’s orders accurately and promptly Engage
in patient and family education concerning health status and
therapeutic regimens Document nursing care and patient status in
accordance with the clinical and system guidelines Collaborate with
other healthcare professionals to ensure optimal patient care
Participate in the review and development of patient care programs
Strong practical skills in nursing and patient care management
Excellent communication and interpersonal skills Capability to work
effectively in a variety of healthcare settings Proficiency in
using healthcare software and patient management systems Medical
Insurance Dental Insurance Vision Insurance 401k Current LPN or LVN
license Basic Life Support (BLS) Certification by the American
Heart Association or equivalent
