Job Description:
Encompass Health is seeking a dedicated LPN-LVN II to join our
dynamic healthcare team, focusing on delivering exceptional nursing
care in accordance with our mission to provide high-quality,
cost-effective care across the post-acute continuum. Working in
both facility-based and home-based settings, this role involves
closely working with patients in 42 states, offering a rewarding
opportunity to make a significant impact on patient recovery and
wellbeing. Administer nursing care to ill, injured, or disabled
patients within facility-based and home-based settings. Monitor and
report changes in patient symptoms or behavior to supervising
registered nurse or physician. Maintain accurate patient health
records and report any discrepancies to the immediate supervisor.
Educate patients and their families on condition-specific therapies
and at-home care procedures. Implement care plans and administer
medications as prescribed by healthcare providers. Assist with the
transfer of patients and provide physical support to assist
patients to perform daily activities. Strong clinical skills with
attention to detail Excellent interpersonal and communication
skills Ability to work both independently and as part of a team
Deep understanding of health and safety guidelines Proficiency in
Electronic Health Record (EHR) systems Medical Insurance Dental
Insurance Vision Insurance 401k Current Licensed Practical Nurse
(LPN) or Licensed Vocational Nurse (LVN) license CPR
certification
