Job Description:
Join H2 Health as a dedicated Outpatient Hand Therapist CHT,
where you will play a crucial role in providing specialized therapy
services. H2 Health is a leader in rehabilitation services,
offering personalized care in both home and facility settings. We
focus on quality treatment plans tailored to individual patient
needs to ensure their fastest possible recovery. This position
involves working closely with patients who have varying degrees of
hand, wrist, and arm ailments, focusing exclusively on their
rehabilitation through a blend of therapeutic techniques. Evaluate
and treat patients with hand and upper extremity conditions.
Develop individualized treatment plans based on patient needs and
physician recommendations. Implement therapeutic interventions to
restore function and prevent disability following injury or
disease. Maintain thorough and accurate patient records and
document patient care services. Communicate with a
multidisciplinary team including physical therapists, occupational
therapists, and physicians to ensure optimal patient care. Educate
patients and their families on therapeutic procedures, treatments,
and home exercises. Keep up-to-date with new techniques and
technologies in hand therapy. Proven ability to perform therapeutic
interventions for the upper extremity. Strong knowledge of tissue
injuries, repair, and rehabilitation. Excellent interpersonal and
communication skills. Ability to design and use custom splints.
Proficiency in patient record management systems. Medical Insurance
Dental Insurance Vision Insurance Life Insurance 401k Must be a
Certified Hand Therapist (CHT). Licensure as an Occupational
Therapist or Physical Therapist in the state practicing.
